Salary: $80,000 /annually
Life Span Range: $80,000/annually - $90,520/annually
Work Location: Los Angeles (Magnolia Place)

Learn more about the Job:

The Case Navigator provides individualized support to families and individuals accessing services through the Community Family Resource Center. This role focuses on assessing client needs, connecting them with appropriate resources, and guiding them through service systems to promote stability, self-sufficiency, and well-being.

Job Responsibilities:

  • Responsible for a wide range of activities designed to implement a family engagement service delivery system.
  • They will communicate with stakeholders, create service plans for participants, and provide referrals and linkages in a timely manner.
  • Recruiting families by canvassing at local sites such as parks and recreation centers, distributing informational flyers at schools and participating in community events.
  • Working with families to conduct assessments, determine service plan goals and provide case navigation and referral linkages.
  • Monitoring service delivery by meeting with the Program Manager and community partners on a weekly basis to discuss cases and by regularly meeting with participants in person or via phone call check-ins.

General

  • Maintain confidentiality in matters of agency operations, personnel, and clients
  • Maintain positive communications with all agency staff, clients, and volunteers
  • Exercise good judgment in performance of duties and responsibilities
  • Support existing agency policies, principles, and mission

Job Qualifications:

  • Bachelor's degree in human services, psychology or related field, or combination of education and experience.
  • Must reside in Supervisorial District 2.
  • One to two years of experience providing case navigation services to children and families.
  • Two or more years of working in the community doing outreach with low-income moderate to high-risk families
  • Languages spoken: English and Spanish required.
  • Ability to work in a multi-cultural environment
  • Good organizational, follow-through and verbal/written communication skills
