Position: Community Engagement Coordinator
Job Type: Part Time (20 Hours Per Week)
Salary: $25/hour.
Work Location: San Juan Capistrano (Community Health Enrichment Collaborative)

The purpose of the Differential Response-Path One (DR-Path One) program is to prevent child abuse and neglect by engaging a greater number of families in services, specifically at Family Resource Center (FRCs) to prevent the unnecessary separation of families by identifying family needs and assisting families in addressing challenges and stressors. Early identification of needs and implementation of FRC services will, in turn, promote the family's ability to become self-sufficient, enhance the well-being of families and their children, and prevent child welfare intervention.

The DR-Path One Community Engagement Coordinator (DRP1 CEC) will develop and implement outreach approaches to engage and establish rapport with DR-Path One families with the goal of enrolling them in preventive services. The DR-Path One CEC is responsible for tracking all outreach efforts and ensuring that all measures are communicated to both the FRC's Coordinator and MDT members. The DRP1 CEC will ensure all data tracking is implemented in an efficient manner.

Job Responsibilities:

  • Organizing, developing, and implementing an outreach plan to engage DR-Path One Families to enroll in preventative services
  • Begin outreach efforts to the family within one (1) business day of receiving referral via telephone, text, e-mail, or in person, as appropriate.
  • Make a minimum of three (3) outreach attempts to engage the family within thirty (30) days, if the first attempt is unsuccessful.
  • Make a final outreach attempt via U.S. mail, after thirty (30) days, if previous contact attempts have not been successful.
  • Assess the appropriateness needs of services.
  • Attend MDT meetings, present cases at MDT meetings, track all outreach efforts, and coordinate and communicate with the FRC Coordinator and MDT members.
  • Attend all required meetings and trainings.
  • Administer all AFK and FaCT-approved measurement tools.
  • Data entry into DRTS, or similar data system, the FaCT database, and Welligent.
  • Performs Community Engagement Coordinator services during FRC operating hours, including two evenings until 8 PM

General

  • Maintains confidentiality in matters of agency operations, personnel, and clients.
  • Ability to work in a multi-cultural environment.
  • Maintains positive communications with all clients, agency and FRC staff and supervisors, partners, funders, volunteers, interns, and visitors.
  • Proficient in MS Word, Excel, PowerPoint, and Outlook.
  • Maintain files, logs, and other records in audit condition as required by the agency or funder.
  • Support agency initiatives related to Trauma Informed, Resilience-Oriented Care (TIROC) and Diversity, Equity, and Inclusion (DEI)
  • Exercises good judgment in the performance of duties and responsibilities.
  • Supports existing agency policies, principles, mission, and Continuous Quality Improvement (CQI) protocols.
  • Weekly schedule includes evenings and some weekends.
  • Other related duties as assigned.


Job Qualifications:

Option One

  • An associate's degree or sixty (60) college units in human services or related field from an accredited college/university; one (1) year of experience, including leadership/supervisory experience, providing direct services to the target population; capable of relating well to individuals from diverse backgrounds, cultures, varied income, and education levels; and computer competency.
  • Proficiency in English is required, and bilingual Spanish or Vietnamese, based on community language needs, is required.
  • Knowledge of public and private social services agencies and community resources, including Federal and State programs.

Or

Option Two

  • Three (3) years of experience, including one (1) year of leadership/supervisory experience, providing direct services to the target population; capable of relating well to individuals from diverse backgrounds, cultures, varied income, and education levels; and computer competency.
  • Proficiency in English is required, and bilingual Spanish or Vietnamese, based on community language needs, is required.
  • Knowledge of public and private social services agencies and community resources, including Federal and State programs.
