The Lead Family Support Counselor is responsible for supervising Infant and Toddler Home Visitation staff, completing other supervisory tasks, and implementing a community-based prevention effort for families. The Lead Family Support counselor will be assigned a reduced caseload of families and will provide in?home and virtual visits using Parents As Teacher curriculum; group parent education classes; case management; and assistance to families to link with resources within the community. The Lead Family Support Counselor may assist families in crisis to assess resources to meet their needs, including coordinating information for participant referrals, compiling and maintaining records, preparing reports and attending all required meetings. The Lead Family Support Counselor will attend Case Management meetings as required.

Job Responsibilities:

Lead FSC Supervisory Tasks

  • Supervise 3 Family Support Counselors (FSCs) and carry a caseload of 9 families. This may vary depending on waitlist and program needs
  • Provide weekly Reflective Supervision to assigned Family Support Counselors
  • Monitor caseloads, enrollments, closures, and PAT compliance of files for assigned Family Support Counselors
  • Provide training and support to all Family Support Counselors
  • Assist with hiring, onboarding, and training of new staff
  • Provide support in Targeted Case Management review and training, quality assurance of data, and reporting
  • Monitor and address performance of assigned Family Support Counselors
  • Complete biannual performance reviews of assigned Family Support Counselors
  • Attend the Home Visiting Collaborative, First 5 meetings, and other meetings as requested
  • In the E3 payroll system, approve timesheets and mileage, complete E3 timesheet corrections, and approve time off requests of assigned FSCs
  • Call and assign referrals when the Secretary/Data Clerk or NRN QA Coordinator is absent
  • Collaborate with community and state partners, maintaining positive working relationships with early childhood partners and providers
  • Assist in conducting biweekly staff meetings when requested
  • Provide support in program evaluation and budget monitoring
  • Complete additional tasks when the Supervisor is absent
  • Complete other duties as assigned by Program Supervisor

Prevention Efforts with Families

  • Provide PAT curriculum, assessments, and screenings to assigned families
  • Teach and model parenting skills classes and provide support in person, at a community site, or virtually
  • Provide participants support and education
  • Conduct Group Connection parenting classes
  • Develop "parent leaders" for Group Connection classes
  • Provide support at other Team Members' Group Connections
  • Set SMART Goals and provide resource connections to assigned families
  • Aid families in crisis situations such as immediate need for shelter, childcare, health care, etc. by providing proper referral and case management
  • Work directly with families to provide basic preventative health care information or to assist with budgeting, home organization, and other skills necessary for improved safety and management.
  • Remain current on community resources to provide proper referrals to families
  • Assist families in identifying/reaching appropriate community resources
  • Advocate for participants as needed; i.e. problems with Cal-Works, Medi-Cal and other systems that participants interact with
  • Translate materials for participants and the agency when needed
  • Attend community meetings and/or agency collaboration meetings as assigned
  • Attend weekly Case Management Meetings as assigned
  • Maintain records as required by the agency, First 5, and Parents As Teachers


Case Management

  • Meets service provision requirements of PAT
  • Meets on-going comprehensive case management requirements and documents in the case file.

Documentation

  • Maintain case files and notes (including consents, releases, etc.)
  • Maintain work and activity documentation, statistics, letters, as required
  • Meet documentation requirements in relation to PAT and program requirements

Community

  • Maintain a high profile in the community
  • Maintain/provide current resource referrals within the community
  • Develop/maintain effective working relations with all individuals and agencies impacting the program within the community
  • Assist Program Supervisor with developing and implementing community outreach programs

General

  • Maintain confidentiality in matters of agency operations, personnel, and clients
  • Maintain positive communications with all agency staff, participants, and volunteers
  • Exercise good judgment in performance of duties and responsibilities
  • Support existing agency policies, principles, and mission.

Job Qualifications:

  • Bachelors Degree in Human Services/Behavioral Sciences from an accredited University or equivalent on the job
  • Two or more years of experience in working with children and families and/or parenting; community outreach with low-income moderate to high-risk families
  • Knowledge of child development and community resources desirable
  • Ability to use diagnostic skills to make assessments
  • Ability to work in a multi-cultural environment
  • Bilingual English/Spanish required
  • Good organizational, follow-through and verbal/written communication skills
